·         Highlights:

Advocate Health Care lowers readmissions with predictive analytics

o   Heart failure readmission rate decreased by 21 percent

o   Algorithm performance 15-20 percent  better than anything that exists in the industry today

o   Patients who received high-risk education had a 20 percent lower readmission rate

o   2016 results show 22 percent overall reduction in readmissions, across all targeted populations
