HEALTH CARE HOME REQUEST FOR INFORMATION

Many of you will be aware of the growth in the Health Care Home programme with 128 practices now using some, or all, of the Health Care Home model of care.  As you’ll see from the message below, the NZ Health Care Home Collaborative members are DHBs and PHOs covering half of the enrolled patient population of New Zealand.  The Collaborative supports its members to implement health care home throughout NZ.

Shared care planning is a key part of the health care home model of care and the Collaborative, working through NZHIT, is requesting information from organisations that provide shared care planning software on whether and, if so, how their solution enables a general practice to meet its requirements in this regard.  Hence, please refer to the information below (and attached) and respond accordingly.

Some important points to note:

  • The Collaborative is not running a procurement process – this is simply an opportunity for providers of shared care planning software to submit information that the Collaborative can use to support its members.

  • The Collaborative’s expectation is for respondents to supply existing documentation/information, in a concise form i.e. the Collaborative is not intending this to over-burden responding organisations with requirements additional to currently available information.

NZHIT’s role is to provide the conduit to get this request for information out to the wider market and ensure its members have an opportunity to respond.  NZHIT is not involved in receiving and collating any responses – these are sent directly to the Collaborative contact as listed in their request below and attached.

REQUEST FOR INFORMATION: SHARED CARE PLANNING SOFTWARE

The New Zealand Health Care Home Collaborative members are DHBs and PHOs covering over half the enrolled patient population of New Zealand. The Collaborative supports these DHBs and PHOs to implement health care home in New Zealand.

Shared care planning is a key part of the health care home model of care (see http://www.healthcarehome.org.nz/Model-of-Care-Requirements, particularly the Proactive Care domain).

The Collaborative is seeking information in order to advise its member DHBs and PHOs on the shared care planning solutions that meet the health care home model of care requirements as they currently stand. To this end, the Collaborative, working through NZHIT, is requesting information from organisations that provide shared care planning software on whether, and if so, how, their solution enables a general practice to:

  • develop care plans collaboratively with a patient, including self-management and clinical management goals, 
  • shared these care plans with other healthcare providers and the patient, and
  • transfer care plans between practices with different PMSs without a loss of content (e.g. GP2GP transfer functionality for care plans).

The Collaborative is working with HISO and recognises the importance of having standards for interoperable shared care plans and solutions that work for health providers and their industry partners, and that will be widely implemented. 

Not a procurement

This request for information is not part of any procurement process. The Collaborative does not procure shared care software, nor does it intend to. DHB and PHO members of the Collaborative may procure shared care software.
