We have attached a Voting Form and a Proxy Voting Form for your use. Voting will take place at the AGM, however if you are unable to attend the AGM you can either complete the voting form and fax it back to the Cluster offices on 04 472 4736 or email it to Renu on r.borst@healthit.org.nz by 5pm Wednesday 26 November 2008. Alternatively you can give your proxy vote to the Chief Executive or another Cluster member to place your vote on your behalf at the AGM.
